Stop by Jensen Jewelers at the Rushmore Mall, fill out the entry, and place it in the entry box. Complete the Sweepstakes entry form with your name, address, phone number, name of the person you love, and their phone number. Listen to Hot 93.1 for prize winner notification. To receive one (1) entry without visiting the store, entrants must mail a self-addressed stamped envelope to Jensen Management, Attn: Kathy Scott, 130 Second Ave. No., Twin Falls, ID. 83301.
Entry must be received by Jensen Jewelers in Rapid City by 2/13/09 12:00 PM to qualify for entry into the Sweepstakes. There will be no limit on mail-in entries, but only one (1) per outer envelope will be allowed for entry requests and submission. No other method of alternative entry will be accepted. All entries become the property of Sponsor and will not be acknowledged or returned.
Any entries that are mutilated, incomplete, or illegible will be disqualified. Mass entries or reproductions will be disqualified. The Sponsor is not liable for any lost or stolen entries.
PRIZES – Grand Prize: Potential winners will be selected at random on 2/13/09 12:00 PM at Jensen Jewlers. Grand prize winners will be notified by Jensen Jewelers store managers. Odds of winning are dependent upon number of eligible entries received. Grand prize winners will receive one (1) $1,000 gift certificate. All taxes and all other expenses not expressly specified herein as part of the prizes are the sole responsibility of the winners. Prizes are non-transferable and may not be transferred or substituted or redeemed for cash. Sponsor reserves the right to substitute a prize for one of equal of greater value. Only the person submitting the form will be eligible for the grand prize.
Potential winners will be notified by phone within two days from the drawing date. Potential winners will be required to complete, execute and return an affidavit of eligibility and liability/publicity release within 10 days of prize notification. In the event that a potential winner cannot be notified within 10 days after notification attempt, or if a potential winner fails to comply with any requirement of these Official Rules, including failure to complete and execute a requested affidavit within 7 days of prize notification, then that potential winner’s prize will be forfeited, and an alternative winner may be selected at random from the remaining eligible entries received. Decision of the judges will be final.
Daily Prize: Daily prizes are $50 gift certificates. Daily winners of the Sponsor’s $50 gift certificate will be drawn by Hot 93.1, on a random basis of selection and time throughout the day. Odds of winning are dependent upon the number of eligible entries received. Both the person submitting the form and those called will receive a daily prize. Daily prize winners are also eligible for the grand prize. Any merchandise returned requires forfeiture of gift certificate or full payment totaling amount of gift certificate awarded. Certificates cannot be exchanged for cash, cannot be used in combination with any other offer, must be used on merchandise of same value or more, and must be used before 2/28/09. Cannot be applied to prior purchase. Need not be present to win.
